Parental/Guardian/Self Authorization
Parental/Guardian/Self authorization are terms and conditions we ask that you read and agree to. By submitting this form you are giving us your *digital signature that you have read, understand and agree to these terms.
1. The CAC of HV reserves the right to cancel this contract if the Director feels the artist/participant is not benefiting from the Program, Exhibition, Reception or is disruptive to others.
2. Permission is hereby granted for photographs and/or video to be taken of my self as a participant and/or my child during Studio & Exhibition activities. Both The Studio and Exhibition Venue have the right to utilize these photographs in printed materials, portfolios, publicity/marketing and art exhibitions.
3. I understand that: (a) The Exhibition fee is non-refundable after our registration deadline. (b) The Artist understands and acknowledges that The CAC of HV does not carry art insurance for any work(s) displayed as a part of the Exhibition. (c) A Hold Harmless Agreement is in effect for the entire event, including “load in” and “load out.” (d) Artist must claim any work(s) within thirty (30) days of the closing date. (e) Children under 16 must be supervised by an adult for Opening Reception(s). (f) Attendance is not required to participate in this event.
