Please make check payable to: The Creative Arts Corner of Hudson Valley. Tuition can be placed in our drop box (located next to our studio door) or by appointment.
Once we know that you and/or the participant will be taking lessons on a regular basis and you are under self-direction, there is an option to make FI arrangements. Please fill out our registration form to start the process and make arrangements with us directly.
Parental/Guardian & Self Authorization
Parental/Guardian & Self Authorization are terms and conditions we ask that you read and agree to. By submitting this form you are giving us your *digital signature that you have read, understand and agree to these terms.
1. The CAC of HV reserves the right to cancel this contract if the Director feels the participant is not benefiting from the program or is disruptive to others.
2. Permission is hereby granted for photographs and/or video to be taken of my self as a participant or my child during activities and the Studio has the right to utilize these photographs in printed materials, portfolios, publicity/marketing and art exhibitions.
3. I understand that: (a) The tuition is refundable until the first day of class sessions. (b) No refunds will be made for absences. (c) Tuition is refundable only according to the written policy guidelines.
