Parental/Guardian & Self Authorization
Parental/Guardian & Self Authorization are terms and conditions we ask that you read and agree to. By submitting this form you are giving us your *digital signature that you have read, understand and agree to these terms.
1. Members must be at least 10 years of age, unless approved by the Director. The CAC of HV reserves the right to cancel this contract if the Director feels the participant is not benefiting from the program or is disruptive to others.
2. Permission is hereby granted for photographs and/or video to be taken of my self as a participating member or my child during club activities. The Studio has the right to utilize these photographs in printed materials, portfolios, publicity/marketing and art exhibitions.
3. I understand that: (a) The membership is refundable until 11:59PM, after my first attended meeting. This applies to the membership fee only. This does not include additional fees such as: events, workshops or field trips. (b) No refunds will be made for absences. (c) Tuition is refundable only according to the written policy guidelines.
